Especially this kind of extra osseous defect, need a great stability of a graft. According to Mellonig, old article however principal of GBR article, said bone graft be used to support the barrier and help prevent collapse into the osseous defect. If you cannot see good tension of membrane, can be collapsed easily and failing easily... 😎😎😎 And how about bone graft substitute? Like to use xenograft? Allograft? Or alloplast? Especially for big GBR and extra osseous defect, I'd like to use autograft and mix with xenograft or allograft. According to Khoury, autogenous bone is still considered as the gold standard, especially first large alveolar ridge augmentation. This case is not Khoury technique however I agree with using autigenius bone for big GBR, and result is different in clinically.
